- Base coverage includes an eye exam every 24 months.
- Helps pay for frames, lenses and necessary contacts.
- Provides up to $200 per eye for laser correction surgery.
- The optional buy-up provides an exam, lenses and frames every calendar year.

Every calendar year, instead of every 24 months. Contact lenses can replace glasses.

Progressive lenses and anti-reflective coating, covered in full in network.

Vision Service Plan (VSP) administers the vision benefit. Medically necessary eye exams go through medical coverage.
