What to send
Spouse
A copy of your registered marriage certificate and your 1040 tax form: the first page, showing filing status, if you have been married more than 2 years. Black out the financial information.
Domestic partner
The Declaration of Domestic Partnership form (Oracle, Help Desk / My Knowledge) and the documents it lists. Their share of the health cost is taken after tax, and imputed income adds to your taxable income.
Children, stepchildren, partner’s children
A copy of the registered birth certificate showing the parents’ names.
Legal ward
Court or legal paperwork granting guardianship or custodianship.

Who can I cover?
Who counts
Children
Up to age 26, married or not, in school or not: your children, stepchildren, adopted or placed-for-adoption children, legal wards, and children covered by a court order (QMCSO).
Your domestic partner’s children
Only if your domestic partner is on your coverage.
Disabled children
Health care coverage, if they meet the requirements in the Summary Plan Descriptions.
Domestic partner
Registered or civil union partners qualify. Otherwise, all of these: both 18 or older; neither married, legally separated nor with another partner; together 12 months or more, exclusive and meant to be permanent; jointly responsible for welfare and finances; same home; not related by blood; each mentally competent. Approved only after you complete a Declaration of Domestic Partnership in Oracle and send its documents.
A child turning 26
Covered through the end of the month they turn 26. That counts as a life event, so they can join their own plan.
Do this
Add them in Oracle during Open Enrollment, then upload their documents by December 16, 2026.
Adding a spouse or dependents, documents not ready
Add them now anyway. Upload the documents in Oracle by December 16, 2026.
