How coverage works

How to start a leave: office hourly and non-bargained plant employees

Request leave at hormel.myleaveproservice.com or call Alight at 855-249-6318. Have your employee ID, two-week schedule, last day worked, expected return date and health care provider contact information ready.

Review your initial packet. Complete the release of information and have your provider complete the medical certification. Return documents by the date Alight gives you: 18 days from the initial packet or first absence, whichever is later.

Track your claim and documents in the Alight portal. Payments begin after approval. Contact Alight on your return date or within 48 hours; follow your site's absence reporting procedure.
